The Crann Centre provides a suite of services and supports to adults, children and families who are living with a neuro-physical disability to enable them to live their best life. The focus is on breaking the pattern of fragmented care services for people with disability and their families. This pattern has marginalised people with disability and their families, increased stigma, and social isolation. Through our innovative services and collaborations we can support people to access services which support them to live their best life. The Crann Centre focuses on measuring impact so to continuously improve what they do and demonstrate value to the people and organisations who fund their work.

Mission

To implement an evidence based, best practice Model of Care that delivers meaningful outcomes for the children, adults and families who use our service and that continue to empower them throughout the whole of their lives

Programme Areas

People living with a neuro-physical disability: all age groups (new-borns, children, teens, adults) and their family.
The Crann Centre's services focus on (but not limited to) Spina, Bifida, Hydrocephalus, Muscular Dystrophy, Cerebral Palsy, Childhood stroke and Spinal Cord Injury.
The Model of Care (based on the Aspen Institute 2 Generation Model) delivers a holistic suite of services for the individual and their family, supporting and empowering them to live their best lives.
The services are organised into six key themes: Mobility, Health&Continence, Social Capital, Psychological, Education, Independent Living
